The continuity chart

Thirty-eight years, six chapters, one chart.

Read the practice the way our clinicians read your history — top to bottom, year by year. Each milestone below folded a new family-medicine service into the care we offer today.

  1. 1987Opens
    Chapter 1 · Founding service

    Annual physicals & preventive care

Dr. Hartwell's father opens the practice with a single rule: every patient gets a full hour for their first visit. The annual physical — blood pressure, screenings, vaccinations, and a real conversation — becomes the spine of how we still practice today. It's why your chart still reads top to bottom.

  2. 1995+ On-site lab
    Chapter 2 · Diagnostics in-house

    On-site lab & bloodwork

No more driving across town to a strip-mall lab for a blood draw. We add a CLIA-waived lab inside the building, so a CBC, lipid panel, A1C, or strep swab happens in the same visit. Results land in your patient portal that same evening — one trip, one co-pay, one chart updated in real time.

  3. 2003+ EHR
Chapter 3 · Charts go digital

    Chronic-care coordination

We migrate every paper chart to a shared electronic record overnight. Suddenly a diabetes panel, a hypertensive neighbour, and an asthmatic teen can be tracked between visits — same week check-ins, refill reminders, and a single care team that actually remembers your last appointment. Coordination, not just treatment.

  4. 2012+ Team of 4
    Chapter 4 · Board-certified team grows

    Women's health & preventive screenings

Dr. Renn joins as the fourth board-certified physician, bringing dedicated well-woman visits, Pap smears, contraception counselling, perimenopause care, and prenatal co-management into the practice. The Cedar Ridge team now covers every stage of life — you never need to be referred out for ordinary women's-health needs.

  5. 2019+ Same-week
    Chapter 5 · Access pledge

    Same-week sick visits

We hold protected same-day and next-day slots open every weekday — a febrile child, a suspected flu, an infected cut, an asthma flare never has to wait weeks. Call before 11 am and we'll almost always see you that day. Telehealth is offered for anything that doesn't need hands-on exam.

  6. todayLive
    Chapter 6 · Today

    Pediatric & teen care

Dr. Okafor leads an expanded child & adolescent program — newborn well-checks, school and sports physicals, vaccinations, ADHD support, acne, and the awkward conversations that teens actually want a doctor for. From the maternity ward to college move-in day, your kid grows up on the same chart you do.

Dr. Eleanor Hartwell, MD

Family medicine · multigenerational panels

Eleanor grew up in this practice — her father opened it in 1987 and she took over in 2009. She maintains some of the longest-running patient panels on Cedar Ridge: three generations of the same family in a single afternoon. Her specialty is the long view: how today's blood pressure becomes tomorrow's heart health, and how a teenager's annual becomes a lifelong chart.

Three generations of our family see Dr. Hartwell — my mother, me, and now my daughter. She remembers our history like she lived it with us.

Margaret L. Cedar Heights · 18 years a patient

I called Tuesday morning feeling awful. Same-week appointment, prescription in hand by noon. I'd never had a clinic move that fast.

David R. Park Lane · same-week sick visit

Bloodwork done in the building, results in my portal by dinner. No second trip, no waiting room at a lab across town. This is how it should work.

For new patients

Plain answers, no phone tree.

01

Can I become a new patient?

Yes — we are actively accepting new patients of every age, from newborns to retirees. Call us on (855) 552-1469 or use the booking form below. Our front desk will confirm your first visit, collect insurance details, and request any records from your previous clinic so your chart is continuous from day one.

02

Do you offer telehealth visits?

Yes. Many follow-ups, medication reviews, lab-result conversations, and minor concerns happen over secure video — you'll get a portal link by text. Anything that needs a hands-on exam (a sore throat, a possible fracture, a blood draw) is seen in person at the office the same week.

03

What if I get sick after hours?

A clinician is on call every evening, weekend, and holiday. Call the main line and follow the prompts — we'll triage over the phone, either book you into the first same-week slot the next morning, or direct you to the right level of urgent care. For true emergencies, always dial 911 first.

04

Which insurance plans do you accept?

We are in network with Medicare, Medicaid, BlueCross BlueShield, Aetna, Cigna, and UnitedHealthcare — most major plans in the region. Our front desk verifies coverage before your first visit and tells you up front what you'll owe. If you're uninsured, ask about our sliding-scale for local neighbours; nobody is turned away for inability to pay.
